Dave Weaver, March, 4/5/99, Flights:302, 303
B737: Departed JFK for DCA scattered 10mi vis with wind shear alert. Took off Rwy 4R, banked left to head SW towards DCA. About 10min out of JFK, the ASSY FLAP became nonfunctional, which was fine until my initial approach into Washington and any extension of flaps made the aircraft very unstable. Weather in DC was 1000ft O/C 3mi vis and rain. Even without usage of flaps I was able to get the aircraft to a safe landing on Rwy 36. Time for this leg: 37:41
B737: Departed DCA for JFK under same 1000ft O/C 3mi visibility, banked left up the river, then right eventually in route to JFK. Flight was fairly uneventful until about 30mi outside NY where the wind shear started picking up. Final approach for 4R was spent fighting the gusting winds to keep on centerline. Nice touchdown, flight time for this leg: 32:34., 1, Problems: ASSY FLAP malfunction during first leg yeilded faster than normal landing speeds.
The ill-fated flight was HI202, and I was about 3 hours into it before the computer locked. Also, I will let HI know before I head off to travel next time.
April, 4/29/99, Flights:
HI624: B737 - Took off from MIA with clear 10mi vis. Leveled off at FL250 and remained in clear skies until 60NM out of MWCR (scattered). Cleared for runway of choice, landed on 08. 1hr 3min.
HI625: B737 - Took off from Grand Cayman at night 6000 broken. Winds calm, nice flight. Landed Rwy 9L in Miami. 58min flight.
HI632: B737 - Took off from Miami under 4K scattered. Immediately lost oil pressure in engine #2. Had to fly a quick pattern around and land for repairs. Took off again after repairs, leveled off at FL230 and landed at Jamaica on Rwy 12. Tricky night landing with no ILS or VASI for assistance! Total this flight: 1hr 10min, 3hours 11 minutes, Problems: Lost oil pressure on last flight.
Flight report for April 1999
HI 42 Honolulu Hana ATR42 32 minutes 122 NM. First attempt, with the Saab 2000 failed for heavy winds with wind-shear and the poor 2000 sinked just in front of the beach. I repeated the flight with the more familiar ATR and without real weather, and was able to do the flight correctly. Followed airway V15, then at Barby intersection with a loose 180 deg. right turn I found myself aligned with Hanas short runway.
HI 43 Hana Honolulu Saab 2000 28 min. 115 NM. Easy jump back home, this time.
HI 50 Honolulu Molokai ATR42 17 min. 52 NM. First flight with new joystick, some difficulties on handling the plane: I know the route by heart, but was confounded, arriving too low, by the two runways and landed on the taxiway.
HI 51 Molokai Honolulu ATR 42 22 min. 68 NM. Elaborate IFR flying with Hapai One departure, airway V8 and the usual angled (304°) approach to runway 26L.
HI 100 Kona Waimea Hilo CRJ Regional Jet 30 min 105NM. Take off from Waimea was rather slow, until I realized that the speed brake was still out! Elaborate IFR approach to Hilo: DME arc then very smooth landing on runway 26 with ILS help.
HI 101 Hilo Waimea Kona CRJ Regional Jet 26 min 96 NM. Easy navigation, smooth landings. Great view.
HI 187 City of refuge Honolulu Twin-Otter 1 hour 1 min. Very funny take-off and sea-landing on the waves.
HI 190 Lihue Princeville Na Pali coast island of Niiahu Barking Sands Port Allen Lihue. Saab 340 37 minutes 145 NM. Sight-seeing flight at 2.000 feet.
HI 101c Kona Hilo B-747-400 15 minutes 58 NM: Very nice night cargo flight: take off from rwy 35, direct Bradshaw NDB at 9000ft, VOR approach, descent and landing on rwy 08 at Hilo. All the above, with a 630.000 lbs. Jumbo, at night, over a restricted area (R-3103) and residential areas. Shall I get a fine from FAA? Its sure!
HI 681 Frankfurt Munich A-320 35 min. 187 NM. SID from rwy 07L, B1 airway, ILS approach on rwy 08L. Smooth landing despite strong winds. The A 320 performs well, just needs some nose-down trim (like the older x-planes)
HI 656 Milan Rome A340 47 min. 295 NM FL 290. Nice night flight, with some gusts of wind (real weather).
HI 657 Rome Milan A 340 51 min. 304NM. Elba 5C SID, airway A1-A9, ILS approach at Malpensa. Excellent instruments panel!

I give below the flight reports of the flights I have made in April. I used Real Weather for all of these flights.
FLIGHTHI30 HONOLULU (PHNL) TO KAHULUI (PHOG)
Take off time 0700 and stopped at the gate at Kahului at 0730.
Total time 30 minutes.
Aircraft 767. Fuel used 2486 lbs. Distance 89 miles.
No problems, nice flight.
FLIGHT HI31 KAHULUI (PHOG) TO HONOLULU (PHNL)
Take off time 0800 and at the gate for 0831.
Total time 31 minutes.
Aircraft 767. Fuel used 3015 lbs. Distance 89 miles.
No real problems, although gusty at Honolulu and was given the wrong runway. Extra fuel to outward trip due no doubt to extra height flown and taxiing at Kahului.
FLIGHT HI01 HONOLULU (PHNL) TO HILO (PHTO)
Take off time 0630 and at the gate at Hilo at 0719
Total time 49 minutes.
Aircraft 737. Fuel used 4931 lbs. Distance 189 miles.
Apart from a rudder fault reported this was a normal flight.
FLIGHT HI02 HILO (PHTO) TO HONOLULU (PHNL)
Take off time 0730 and at the gate at 0820.
Total time 51 minutes
Aircraft 737. Fuel used 6278 lbs. Distance 189 miles.
Thanks to a wrong vectoring by ATC and being turned on ILS on to a non existant runway I had to do another circuit and eventually landed on runway 08L. Otherwise uneventful.
FLIGHT HI180 KONA TO KONA (PHKO)
Take off time 0930 and disembarked passengers at the gate at 1050.
Total time 1 hour 20 minutes.
Aircraft ATR42. Fuel used 854 lbs. Distance travelled approximately 130 miles.
Pleasure flight around volcano and crater. I hope I did not give them too long a trip.
FLIGHT 200c KONA (PHKO) TO MIDWAY (PMDY)
Take off time1715 and came to a halt at the gate at 2005.
Total time 2 hours 50 minutes.
Aircraft 747. Fuel used 93025 lbs. Distance travelled 1366 miles.
Uneventful flight.
FLIGHT 201c MIDWAY (PMDY) TO KONA (PHKO)
Take off time 2230 and at the gate at Kona at 0135.
Total time 3 hours 5 minutes.

Uneventful flight. Took off in storm at Midway.
FLIGHT HI480 CHICAGO (KORD) TO SAN FRANCISCO (KSFO)
Take off time 0800 and at the gate at San Francisco at1237.
Total time 4 hours 37 minutes.
Aircraft 737. Fuel used 25555 lbs.(economical!). Distance 1656 miles.
The flight was uneventful except for high winds for a few hundred miles enroute. However I was directed to the wrong runway (there wasn't one!) and had to go around again for runway 28R. High wind on landing and considered diverting, but all OK, except for a very hard landing.

Take off time 0645 and at the ramp at Kahalui at 0711.
Total time 26 minutes.

All flights uneventful for the most part. Flights in/out of OGG seem to have alot of wind shear alerts. I noted this on all my flights there.
Flight from LAX to HNL was long.
HI451: Fresno-SFO, 737, direct route at 15,000 ft, time: 39m, fuel:3643lbs

Problems:
HI84 lost oil pressure in #2 engine, landed at Lihue with substantial
crosswind. Applied reverse thrust and forgot I only had one engine, I drifted
off the side of the runway and eventually stopped on the taxiway running
parrallel to the runway. Landing gear sustained minimal damage.
